HSS Perform

HSS Perform is a program that offers guided access to expert musculoskeletal care from a team of dedicated medical professionals, including surgeons, physicians, physician assistants, physical therapists, and nurse practitioners. HSS Perform will guide you through the process of getting care at HSS including surgical care, physical therapy, sports performance and training and other services to enhance your health and wellness.

  • Back or joint pain
  • A sports or orthopedic injury
  • A need for physical therapy
  • A recommendation to consult an orthopedic specialist
  • A recommendation for orthopedic surgery and want a second opinion

HSS Florida is an in-network provider for those team members enrolled in one of the Blue Cross Blue Shield health plans offered by The Breakers.

The team's hours of availability are Monday - Friday, from 9:00 a.m. to 5:00 p.m. ET. Messages left outside of these hours of operation will be returned the next business day. Family members will need to state their relationship to the employee.

No, you do not need to enroll in HSS Perform program to receive care at HSS. All employees and family members automatically have access to utilize HSS services.

Insurance coverage for clinical care is subject to insurance verification.

There is no cost to you for utilizing the HSS Perform program. Most clinical care services (including but not limited to, physical therapy or physician visits) are billable to your insurance, and standard out-of-pocket costs will apply. Please contact your insurance carrier with specific questions about coverage for your care.

All employees and their family members have access to HSS at no additional cost beyond the standard copays, coinsurance and deductibles for the medical services received from HSS. Family members include, but are not limited to, spouse, partner, children, parents-in-law, grandparents, aunts, uncles, and cousins, subject to insurance verification.

HSS is a participating provider in many health plan networks, including your employer sponsored health plan. Your billing statement and charges are dependent on the type of care you receive, your physician and the amount of coverage from insurance or financial assistance.
