HSS Care Link is a web portal that allows HSS Rehabilitation National Network Members & Collaborative Partners to securely view patient medical information as well as exchange secure messages with our physicians and clinical teams to coordinate patient care.
As a reminder to maintain your access each designated “Site Administrator” are responsible for Site Verification & Managing your clinic. Every month, upon login, site administrators will be prompted to verify all users at their site. If any users have left the clinic/facility or no longer require access to Care Link, their status should be changed to “no longer active.” In addition, each individual Care Link user must log in, at least once, every six months in order to maintain their Care Link credentials.
Technical Difficulties
Please call the designated “Care Link” tech support line at 646.989.1701 if you experience issues with:
- Lost Dual Authentication
- Login Blocked
- Expired Password
