How to Get an Appointment

The first step in getting an appointment at HSS is to contact us via telephone, email, fax, or regular mail.

Or, you can fill out our online International Center Appointment Request Form to let us know your basic information as well as a brief description of your medical condition. You will need to provide us with your demographic information, such as your address, telephone, email and date of birth. In addition, please send us your diagnostic films (x-ray, MRI, etc.), along with your medical reports (in English) and any other relevant information on your case.
