Case Management
Case Managers are an integral part of the HSS health care team. They specialize in providing our patients with:
- Assistance with discharge planning and coordination of medical needs and, if indicated, referrals for physical therapy, rehabilitation, or home care programs after leaving the hospital.
- Emotional support and counseling for patients and family members.
- Consultation regarding concerns about coping with illness and/or disability.
- Exploration of resources to assist with personal, financial, and social needs which may have been affected as a result of your need for surgery.
Our Case Managers can help you before, during, and after your hospital stay. We have a team of certified social workers and registered nurses who assist patients from pre-admission to post-discharge.
